Disability support available

We offer a range of services for young people with a disability and their carers as part of our Home and Community Care Program for Younger People.

The program provides support for younger people with disabilities whose capacity for independent living is at risk and:

  • are under 65 years of age (or under 50 for individuals from an Aboriginal or Torres Strait background).
  • have functional limitations affecting their ability to manage day-to-day activities
  • need support to live independently at home, or while out in the community.

Services are targeted to people with moderate, severe, or profound disabilities and their unpaid carers.

Services available

  • showering and dressing
  • meal preparation
  • occupational therapy
  • social support groups
  • creating a safe home environment
  • respite care at home
  • domestic assistance
  • carer groups.

You can contact us for services for yourself, or on behalf of someone else:


Fees are based on:

  • the services you require
  • your income
  • your capacity to pay (services will still be offered even if you are assessed as having no capacity to pay). 

Next steps

  1. When we receive the referral, a Council Intake Officer will visit you at home to talk about your needs and goals.
  2. If you are eligible to receive services, a support plan will be created that includes your goals and support preferences. 
  3. A Council Intake Officer will then contact you to discuss your support plan; any costs involved; and a start date.
