Request Change of Details: Name and Address

Use this form to notify Council of a change of personal details, including legal name, phone number, email address and mailing address.

To notify Council of a deceased resident, provide authority, provide proof of executorship of an estate, change property ownership, or to add/remove ratepayer please complete Request Change of Details: Third-Party Authority and Rates.

Step 1.Supporting documents

  • If you have changed your name provide Marriage certificate or official name change document from the Registrar of Births, Death and Marriages
  • If reverting back to maiden name provide a copy of your Birth certificate and Marriage certificate
  • If property is owned in a company name provide Proof of Directorship
