Request Change of Details: Name and Address

Use this form to notify Council of a change of personal details, including legal name, phone number, email address and mailing address.

To notify Council of a deceased resident, provide authority, provide proof of executorship of an estate, change property ownership, or to add/remove ratepayer please complete Request Change of Details: Third-Party Authority and Rates.

Step 1.Supporting documents

If you have changed your name provide Marriage certificate or official name change document from the Registrar of Births, Death and Marriages
If reverting back to maiden name provide a copy of your Birth certificate and Marriage certificate
If property is owned in a company name provide Proof of Directorship
