Request Change of Details: Name, Address, Phone, Email, Registration

Use this form to notify Council of a change of personal details, including legal name, phone number, email address and mailing address for the purposes of updating your Rates, Animal Registration or Vehicle Registration (Parking Permit).

To notify Council of a deceased resident, provide authority, provide proof of executorship of an estate, change property ownership, or to add/remove ratepayer please complete Request Change of Details: Third-Party Authority and Rates.

Step 1.Supporting documents

  • If you have changed your name provide Marriage certificate or official name change document from the Registrar of Births, Death and Marriages
  • If reverting back to maiden name provide a copy of your Birth certificate and Marriage certificate
  • If property is owned in a company name provide Proof of Directorship

Step 3.Submit completed form and supporting documents

Required documents

  • If you own property in Maroondah you will need to provide the Property Assessment number found on your rates notice for a mailing address or name change request. 
  • If you have a pet/s registered in Maroondah you will need the Kiosk and Tag numbers found on your Animal Registration notice to make any updates to your details. 

You will also need the following documents or information to request a:

  • Change of mailing address - Assessment number and Property Address required if you own property in Maroondah.
  • Change of name - Marriage certificate or/ Official name change document from the Registrar of Births, Death and Marriages required
  • Change of vehicle registration details - Vehicle registration papers required

If you are acting on behalf of someone else:

Please provide the necessary documentation if you are acting in the capacity of:

  • Power of Attorney or
  • Executor/Executrix of a deceased estate.
