Parents/guardians will receive a consent card from their child’s school. The card must be completed and returned to the school in early Term 1.
Whether you are consenting or not consenting for your child to have the immunisations Council requires the consent form to be completed and returned.
You can complete the consent electronically below or return the consent form to the school or email it to immunisation.health@maroondah.gov.au.
